Storm ID
About
Storm ID developed its digital health platform to securely combine patient-generated health data from devices, sensors and wearables with healthcare data held in clinical systems. Bringing these data sources together enables AI and machine learning to generate new insights and support the modernisation of care pathways.
Through electronic health record integration and clinical dashboards, connected data can be used to develop and train algorithms. These algorithms interpret data, prioritise risk and provide decision support to care teams, helping to increase patient engagement, improve outcomes and reduce costs for healthcare organisations.
The platform can underpin virtual care services across long-term condition pathways, including digital diagnostics, virtual wards, remote patient monitoring and patient-initiated follow-up. Storm ID’s COPD, heart failure and virtual appointment services have been used in the NHS, supporting the transition of care from acute settings into the community by reimagining and co-designing end-to-end clinical pathways with healthcare professionals and patients.
The platform has an open architecture and has been designed with privacy at its core, empowering patients to control their data and authorise who – and what – they share it with. It supports standards and requirements including HL7 FHIR, DTAC, DCB0129 and DCB0160, and is certified under the Medical Device Regulations.
