About
CogStack is an open-source clinical AI and data platform making the unstructured information locked in health records (clinic letters, discharge summaries, radiology and pathology reports) usable for direct care, operational planning and research.
Around 80% of the clinical record is free text. CogStack ingests it, extracts and codes clinical concepts to international standards (like ICD10 and SNOMED CT), so that healthcare organisations can search, cohort, report and build on their own data without exporting it.
The platform runs entirely within the organisation’s secure environment (on-premise or in hospital’s own cloud tenancy) so patient data never leaves the organisation’s control. CogStack interoperates via HL7, FHIR and OMOP, and works alongside existing EPRs rather than replacing them. We have developed tools which work within the EHR environment as well as interfaces and reporting systems independent of operational systems.
CogStack is deployed at a number of NHS trusts, including King’s College Hospital, University College London Hospitals and Guy’s and St Thomas’, is integrated into Secure Data Environments (SDEs) and at sites across Europe, Asia and Australasia.

CogStack has developed from a research-focussed solution developed for the 100,000 Genomes project to a broad clinical analytics and information platform that works across clinical settings regardless of EHR systems and data architecture. From supporting clinical coding teams at Guy’s and St Thomas' NHS Foundation Trust to improve coding accuracy and recover lost revenue, to supporting life sciences organisations in ground-breaking research, CogStack delivers ROI to customers through rapid deployment, low infrastructure and compute costs and accurate, trusted results. We are currently deployed at seven NHS trusts, working with several Secure Data Environments and partnering with industry to support in making clinical data safely available to clinicians and researchers to improve patient outcomes. CogStack was recently listed by the National Institute for Health and Care Research (NHR) as one of 20 game changers which has supported the development of life-changing technology over the past 20 years.
